Bridging the gap between inpatient and independence…

Completing inpatient treatment is just the first step on the road to recovery. Our Intensive Outpatient Transitional Program helps patients and families transition from structured treatment facilities to a safe and supportive outpatient setting. Our goal is to successfully return patients to home, work, school and family life while continuing the healing process.

As patients develop greater independence and self-care, we reduce the intensity of services while continuing to provide support. Patients learn how to live without self-destructive behaviors, rebuild their lives, rediscover the true self and confidently share that person with the world.


About Our Intensive Outpatient Transitional Program

We provide intensive therapeutic services to individuals recovering from:

Treatment components and durations are individualized to meet the needs of each patient and family. Initial treatment plans are determined in consultation with the patient’s primary care providers and or inpatient specialists. Follow-up assessments and adjustments are performed regularly.
